Licensed Social Worker

About the Role

The Licensed Social Worker (LSW) oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current Federal, State, and local standards. The LSW works directly with discharge planners, family, or responsible parties to initiate tours of the facility and follow-up procedures.

Responsibilities

• Assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility.
• Develop and manage initial contact with discharge planners, family, or responsible parties to initiate tours and follow-up procedures.
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of patients/residents.
• Develop and maintain a good working rapport with intra-department personnel, other departments, and outside community health, welfare, and social agencies.
• Keep abreast of current federal and state regulations, as well as professional standards.

Qualifications

• Must have a current Social Work license, in good standing with the state (as required).
• Bachelor’s Degree in Social Work strongly preferred.
• Must have at least 3+ years of experience in a long-term care facility.
• Must be able to organize and prioritize multiple tasks effectively.
• Skilled communicator, director, and motivator.
• Reliable transportation required.

Pay

USD $35.00 - USD $37.00 /Hr.

The wage range for this role takes into account the wide range of factors considered in making compensation decisions, including skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
