About the role
Seeking a Licensed Social Worker with 2-3 years’ long-term care experience to provide comprehensive psychosocial services in a hospice setting.
Responsibilities
- Conducting initial psychosocial evaluations and ongoing counseling for patients and families
- Providing direct casework, bereavement support, and community education
- Making appropriate referrals and collaborating as a member of the interdisciplinary team
Qualifications
- Must hold an active license (LSW, LCSW, or equivalent)
- Experience in long-term care and hospice services preferred
Skills
- Strong communication, empathy, and teamwork skills
Pay
