About the role
We are seeking per diem Licensed Social Workers to work weekday evenings (5:00 pm–8:00 pm) and weekends as needed in a nursing home setting, reporting to the Director of Social Work.
Responsibilities
- Deliver quality social work services to patients, residents, family members and significant others.
- Coordinate the discharge planning process.
- Participate in interdisciplinary team meetings and quality improvement activities.
- Provide counseling and ensure satisfaction of patients, residents and family members.
Qualifications
- Master’s Degree in Social Work.
- Active licensure in New York State.
- Prior experience in a long‑term care, rehabilitation or hospital setting (experience with the elderly preferred).
- Strong communication skills.
- Ability to work collaboratively across disciplines.
- Familiarity with electronic medical records (EMR).
