Responsibilities
- Affords assistance to the physician and other members of the care team regarding significant social and emotional factors related to the patient's health issues.
- Assesses social and emotional factors within five days of admission to gauge the patient and family's ability to cope with daily living and terminal illness.
- Supports the patient and family in understanding, accepting, and following medical recommendations, and offers services to aid in optimal social and health adjustment.
- Utilizes available resources, including family and community agencies, to help the patient and family adapt to the limitations of the illness.
- Monitors, documents, and reports changes in social and emotional conditions.
- Participates in the interdisciplinary team and contributes to the creation and evaluation of the patient's care plan.
- Engages in ongoing education and attends committee meetings as needed.
- Completes required documentation in a timely manner according to Good Shepherd Hospice policies and procedures.
- Supports staff with handling grief and addressing patient/family issues.
- Maintains confidentiality regarding patient and family statuses.
Qualifications
- Master’s degree in social work from a Council on Social Work Education (CSWE)-accredited program, or
- Bachelor’s degree in social work from a CSWE-accredited program, supervised by an MSW, or
- Bachelor’s degree in psychology, sociology, or another field related to social work, supervised by an MSW,
- Licensure to practice in the state where care is provided, if required,
- At least one year of experience in a healthcare setting,
- Demonstrated commitment to hospice philosophy of care,
- Ability to pass background checks and drug screens.
