Licensed Social Worker

Duties & Responsibilities

• Aid in the planning, development, organization, implementation, evaluation, and direction of the Social Services Department within the facility.
• Collaborate with discharge planners, family or responsible parties to initiate tours of the facility and follow up on procedures.
• Contribute to the creation and execution of policies and procedures addressing the medical, social, and emotional needs of patients/residents.
• Maintain positive relationships with internal department staff, other departments within the facility, and external community health, welfare, and social agencies.
• Stay informed about current federal and state regulations and professional standards.

Qualifications & Requirements

• Hold a current Social Work license in good standing with the state.
• Preferred to have a Bachelor’s Degree in Social Work.
• At least 3+ years of experience in a long-term care facility.
• Capable of managing multiple tasks effectively and maintaining a good rapport with various stakeholders.
• Excellent communication, leadership, and interpersonal skills.
• Must possess reliable transportation.
