Responsibilities

• Conduct psychosocial assessments and lead care planning.
• Coordinate safe discharges and connect residents with community resources.
• Collaborate with nursing, therapy, and medical teams to address emotional, social, and financial needs.
• Advocate for residents’ rights and guide families through transitions.
• Ensure delivery of dignified, goal-focused, person-centered care in a homelike environment.

Qualifications & Skills

• Licensed Social Worker credential.
• Strong interpersonal and communication skills.
