Licensed Practice Nurse

Job Summary

The Licensed Practice Nurse assesses and enhances the health status of the persons served in 5 residential programs located in Crystal, Golden Valley, and Coon Rapids areas. They provide expertise regarding health care to staff, persons served, and members of the interdisciplinary teams. Hours are flexible, including on-call rotation.

Responsibilities

• Maintain health record systems
• Engage in daily health discussions with individuals served and their staff
• Serve as a liaison within the facility, health care providers, and members of the interdisciplinary teams
• Attend staff meetings and in-service classes as required
• On-call triage

Requirements

• Licensure as a LPN
• Valid driver’s license and proof of valid insurance

Schedule

Monday through Friday, 7:30AM to 4:30 PM
