Licensed Practical Nurse-Telehealth & Outside Provider Liaison
Job Duties
Assists in providing access to healthcare by facilitating in-house provision of both telemedicine technology and outpatient services.
This position coordinates and conducts virtual patient appointments via secure telehealth platforms to include but not limited to phone, video, or chat.
Ensuring timely check-ins, clear communication of patient status and coordination follow-ups.
Scheduling of appointments.
Care coordination and collaboration with internal and external providers.
Serve as the primary contact for external providers.
Documentation within the electronic health record (EHR).
Provide in clinic support to outside providers to include but not limited to Audiology, Optometry, Podiatry, Gastroenterology, and X-ray services: x-ray, ultrasound and echocardiogram.
Minimum Qualifications
- Clinical knowledge of principles and practices of practical nursing, medication administration, psychiatric disorders and developmental processes.
- Ability to implement planned nursing care and provides guidance to other staff.
- Ability to work shifts; working overtime hours or shifts as required providing care.
- May be exposed to aggressive residents.
- Va LPN licensure
Additional Considerations
Previous work experience in a psychiatric work environment preferred.
