Responsibilities
- Provide resident-centered care in a skilled nursing and rehabilitation setting.
- Administer medications and treatments, monitor resident conditions, and document in EMRs.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans.
- Support rehabilitation and long-term care goals.
- Educate residents and families about care processes.
- Promote dignity, safety, and comfort in a respectful, family-like environment.
Qualifications & Skills
- Valid LPN license.

- Ability to monitor and respond to resident needs effectively.
- Proficiency in using EMRs and administering treatments.
