About the role

A Licensed Practical Nurse (LPN) serving as a patient navigator, responsible for guiding new patients into the program, providing education, and ensuring continuity of care. The role involves clinical support to both providers and patients, facilitating diagnosis and treatment processes, and collaborating with healthcare teams.

Responsibilities

• Assess patient needs via phone or documentation review.
• Coordinate patient care, activities, and support services.
• Serve as a communication link between patients and providers.
• Develop and implement patient screening tools and navigational procedures.
• Handle patient concerns related to appointments, communication, and billing.
• Maintain medical records, clinical supplies, and equipment.
• Provide limited patient and family education in person or by phone.
• Support clinical procedures and assist physicians.

Qualifications

• Minimum of 2 years LPN experience.
• Cardiovascular and adult echocardiography experience preferred.
• Graduate from Specialty Training Program – Nursing.
