About the role

A Licensed Practical Nurse serving as a patient navigator, guiding new patients, providing education, and ensuring continuity of care. This role supports clinicians and patients by facilitating diagnosis, treatment, and patient engagement through phone and in‑person communication.

Responsibilities

• Utilize clinical knowledge to assess patient needs and facilitate program entry.
• Perform evaluations via phone or documentation review and document findings.
• Collaborate with healthcare providers to coordinate and implement care plans.
• Communicate with patients through various channels and ensure relevant information is available for providers.
• Coordinate patient activities, support services, and educational programs.
• Maintain follow‑up systems, develop screening tools, and improve navigational procedures.
• Address patient concerns related to appointments, communication, and billing.
• Conduct medical record reviews, provide nursing care, and assist physicians.
• Ensure equipment is operational and maintain clinical environments.

Qualifications

• Minimum 2 years of LPN experience; cardiovascular and adult echocardiography experience preferred.
• Graduate from a specialty nursing training program.
• Strong communication, collaboration, and organizational skills.
