Licensed Practical Nurse - Nurse Navigator - Adult Echocardiography

About the role

Seeking a Licensed Practical Nurse to serve as a patient navigator, guiding new patients, providing education, and ensuring continuity of care. The role involves clinical support to providers and patients, facilitating diagnosis and treatment, and collaborating with healthcare team members. This full-time position typically involves working in a clinical environment, adhering to organizational policies, and contributing to patient care excellence through teamwork and continuous improvement.

Responsibilities

• Utilize clinical knowledge to facilitate patient program entry and evaluate patient needs primarily through phone or documentation review.
• Coordinate care plans, communicate with patients, and serve as the main contact for providers.
• Assist in scheduling, develop patient screening tools, and improve navigational procedures.
• Address patient concerns, maintain clinical supplies, and review medical records for physician reporting.
• Provide nursing care, monitor equipment, and maintain exam and treatment areas.

Qualifications and Skills

• Minimum 2 years of LPN experience; cardiovascular and adult echocardiography experience preferred.
• Graduate from a Specialty Training Program in Nursing.
• Strong communication, collaboration, and organizational skills.
