Licensed Practical Nurse - Adult Echocardiography

About the role

Role Summary: A Licensed Practical Nurse serving as a patient navigator, guiding new patients into the program, providing education, and ensuring continuity of care. The role involves clinical support, collaboration with healthcare providers, and assisting in diagnosis and treatment processes.

Responsibilities

• Facilitate patient introduction to the program through clinical knowledge and documentation review.
• Coordinate and communicate with patients via phone, mail, and in person regarding their care needs.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Serve as a point of contact for providers, scheduling, and patient activities such as support groups and educational events.
• Perform patient assessments, education, and follow-up, ensuring timely surveillance and care continuity.
• Maintain clinical supplies, conduct medical record reviews, and assist physicians and procedures.
• Inspect and maintain clinical equipment and exam rooms to ensure proper operation.

Qualifications

• Minimum of 2 years LPN experience; cardiovascular and adult echocardiography experience preferred.
• Graduate from a Specialty Training Program in Nursing.
• Strong communication, organizational, and collaborative skills.
• Ability to perform patient evaluations, education, and support activities efficiently.
