Licensed Practical Nurse - Outpatient Clinics

About the role

The LPN assigned to PACT/Medical Specialty clinics delivers direct patient care to adult through geriatric Veterans for both acute and chronic diseases in an outpatient clinic setting. They recognize age-related needs and demonstrate appropriate care, considering the cognitive, physical, emotional, and chronological maturity of patients.

Responsibilities

• Recognizes age-related needs and provides appropriate care
• Considers cognitive, physical, emotional, and chronological maturity of patients
• Recognizes lifestyle differences and respects patient values, beliefs, and practices
• Performs health history screenings
• Screens, intervenes, and follows through for acute/chronic medical and emotional conditions
• Performs patient education and documentation for prevention, healthier lifestyles, functionality, and disease management
• Observes and reports changes in patients' physical and emotional conditions
• Affordably assesses and intervenes in both acute and chronic medical conditions
• Delivers patient care in a timely manner according to approved policies and procedures
• Demonstrates ongoing improvement as an experienced LPN

Requirements

• Graduate of an approved school of practical or vocational nursing
• Full, active, current, and unrestricted licensure as an LPN/LVN in a state, territory, or commonwealth of the United States, or District of Columbia
• One year of successful practice as an LPN/LVN or equivalent
• Technical proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations
• Knowledge and ability to appropriately carry out assigned patient care based on the patient's condition
• Knowledge and ability to use judgment in selecting the appropriate order and sequence of procedures and treatments
• Knowledge and ability to accurately recognize and document relevant patient information
• Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance, and initiate appropriate emergency interventions
• Knowledge and understanding of human behavior, patient motivations, and reactions to situations
• Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures
• Ability to organize and deliver care, recognizing and considering emotional, cultural, spiritual, socio-economic, and age-related factors
• Ability to prepare and administer prescribed medications and perform treatments according to established policies and procedures
• Ability to observe and document patient's response to medications and report any deviations from normal to an RN or MD/DO
• Ability to establish constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication, and socialization skills
• Ability to foster an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery
• Ability to incorporate knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers
• Ability to actively seek out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice

Qualifications

• Education: Graduate of an approved school of practical or vocational nursing
• Licensure: Full, active, current, and unrestricted licensure as an LPN/LVN in a state, territory, or commonwealth of the United States, or District of Columbia
• Experience: One year of successful practice as an LPN/LVN or equivalent
• Technical proficiency: Knowledge and ability to initiate, perform, and complete assigned duties in providing care to variable patient populations
• Knowledge and ability: Judgment in selecting the appropriate order and sequence of procedures and treatments, accurate recognition and documentation of relevant patient information, recognition of urgent or emergent patient care situations, seeking assistance, initiating appropriate emergency interventions, and understanding human behavior, patient motivations, and reactions to situations
• Skills: Ability to prepare and administer prescribed medications and perform treatments according to established policies and procedures, observe and document patient's response to medications, report any deviations from normal to an RN or MD/DO, establish constructive relationships with individual patients and their families, foster an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery, incorporate knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers, actively seek out educational opportunities to enhance nursing knowledge and skills, and share new knowledge gained with other staff to improve and advance nursing practice

Pay

Competitive salary and regular salary increases when setting pay. A higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA.
