Position Information
Location: Brick, New Jersey
Schedule: Full-Time | Sunday-Thursday | 3:00 PM - 11:00 PM
Starting Pay: $36.10 per hour
Additional Shift Differentials: Earn an additional $2.00-$4.00 per hour based on applicable shift assignments.
What You'll Do
- Provide Clinical Nursing Care
- Deliver nursing services within residential group homes while promoting the health and well-being of individuals receiving services
- Monitor changes in health status and respond appropriately to medical concerns and emerging healthcare needs
- Absorb changes in health status and respond appropriately to medical concerns and emerging healthcare needs
- Assist in developing, implementing, and evaluating individualized healthcare plans that support each person's unique medical needs
- Cook up comprehensive care work closely with the agency Registered Nurse, residential leadership, physicians, specialists, pharmacies, and interdisciplinary team members to ensure coordinated healthcare services
- Accompany individuals to physician appointments, hospital visits, rehabilitation admissions, specialty consultations, and other medical appointments when needed
- Advocate for individuals by ensuring continuity of care and effective communication among healthcare providers
- Support Medication & Health Management
- Maintain medication administration records, health assessments, and other clinical documentation with accuracy and attention to detail
- Support medication administration processes while ensuring compliance with agency policies, physician orders, and healthcare regulations
- Aid residential staff with health-related education, guidance, and nursing support as appropriate
- Promote Wellness & Quality Care
- Encourage healthy lifestyles and proactive healthcare practices that enhance quality of life
- Participate in quality assurance initiatives, health education, and ongoing clinical training
- Help maintain safe, healthy residential environments where individuals can thrive while receiving person-centered care

Qualifications
- Must be at least 18 years of age
- Graduate of an accredited School of Nursing required
- Current Licensed Practical Nurse (LPN) license issued by the New Jersey Board of Nursing required and must remain in good standing
- Minimum of one (1) year of experience as a Licensed Practical Nurse required
- Valid New Jersey driver's license required with no more than five (5) motor vehicle points
- Strong communication, documentation, organizational, and clinical assessment skills
- Previous experience working with individuals with intellectual and developmental disabilities is preferred but not required
