Licensed Practical Nurse (LPN) – Principal Care Management (PCM)

Responsibilities

• Review patient charts, medications, labs, and care plans
• Care coordination with providers, nursing staff, pharmacies, and families when needed
• Accurate and thorough documentation of patient interactions
• Identification and escalation of changes in condition
• Chronic disease management and patient education support
• Ensuring compliance with CMS guidelines for PCM services

Qualifications

• Active LPN license required
• Nursing home or long term care experience preferred
• Experience with chronic care management or PCM is a plus
• Strong communication and organizational skills
• Comfortable working independently in a remote environment
• EMR experience preferred
