Licensed Practical Nurse (LPN) - Part Time Evening

About the role

We are seeking a compassionate LPN to provide direct nursing care in a resident-centered facility.

Responsibilities

• Administering medications, wound care, and skilled nursing procedures.
• Assisting residents with activities of daily living, ensuring their comfort, dignity, and safety.
• Supervising nursing assistants and medication aides during shifts.
• Monitoring residents' physical and emotional well-being and notifying physicians of any changes.
• Developing, implementing, and reviewing individualized care plans in collaboration with the interdisciplinary team.
• Completing accurate clinical documentation and coordinating admissions, transfers, and discharges.
• Participating in quality assurance, staff education, and regulatory activities.
• Performing additional duties as assigned by leadership.

Requirements

• Active LPN/LVN license; CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care and supervisory experience preferred.
• Strong clinical, observation, medication, and documentation skills.
• Knowledge of long-term care regulations and proficiency with electronic health records and Microsoft Office.
• Excellent communication, independent working ability, and sound clinical judgment.
