About the role
Licensed Practical Nurse (LPN) providing professional nursing care in a long-term care setting, under supervision of registered nurses, advanced practice providers, or physicians.
Responsibilities
- Assess residents' health, administer medications and treatments as ordered, and ensure their safety, comfort, and well‑being.
- Perform testing, recognize problems, recommend solutions, and respond effectively to emergent situations.
- Document resident interactions, care outcomes, medication, behavior, activity, and dietary information.
- Collaborate with healthcare team members, communicate effectively, and maintain confidentiality.
- Counsel residents and families on prevention and treatment plans, and develop self‑care education programs.
- Contribute to resident assessments and provide technical support to healthcare professionals.
- Perform additional duties as assigned.
Qualifications & Skills
- Graduate from an accredited practical nursing program; associate degree preferred.
- Current unencumbered LPN license with state nursing board or multi‑state privileges.
- Experience with supervised clinical practice or at least 400 hours as an LPN.
- Knowledge of resident care principles, medication management, and, if applicable, IV therapy skills.
- Ability to build effective relationships, communicate clearly, and handle sensitive information with confidentiality.
- Completion of required continuing education and department‑specific competencies.
