Monarch Springs Wellness & Rehabilitation in University City, MO is seeking a compassionate and dependable Licensed Practical Nurse (LPN) for the Night Shift (7:00 PM–7:00 AM). Join our supportive nursing team in a skilled nursing and rehabilitation setting.

About the Role

As a Licensed Practical Nurse (LPN), you will provide direct nursing care to residents while collaborating with RNs, CNAs, physicians, families, and other healthcare team members. Your role includes administering medications, monitoring resident conditions, maintaining documentation, and delivering resident-centered care.

Responsibilities

  • Provide safe, compassionate, and high-quality nursing care to residents.
  • Administer medications and treatments accurately and on time according to physician orders, care plans, and facility policies.
  • Monitor residents for changes in condition and promptly report changes to the appropriate nurse or provider.
  • Perform nursing assessments and obtain vital signs as directed.
  • Communicate resident condition changes to physicians, RNs, nursing leadership, and other members of the interdisciplinary team.
  • Provide direct supervision and support to Certified Nursing Assistants (CNAs).
  • Participate in developing, implementing, and evaluating individualized resident care plans.
  • Complete accurate and timely clinical documentation and medical records.
  • Provide education to residents and families regarding medications, treatments, care plans, and health needs.
  • Follow infection prevention and infection control policies and procedures.
  • Participate in the facility's Quality Improvement Program.
  • Follow established standards of nursing practice, facility policies, and applicable state and federal regulations.
  • Communicate important resident information to other departments, including room changes, hospital transfers, leaves of absence, admissions, and discharges.
  • Collaborate with physicians, therapists, social services, dietary staff, CNAs, RNs, and other healthcare professionals.
  • Promote resident dignity, safety, independence, and quality of life.
  • Respond to resident needs and call lights in a timely and professional manner.
  • Perform other nursing duties as assigned by the Director of Nursing or nursing supervisor.

Requirements

  • Current, active Missouri LPN license required.
  • Graduate of an approved practical nursing program.
  • Current CPR/BLS certification preferred or required according to facility policy.
  • Previous skilled nursing, long-term care, nursing home, rehabilitation, or post-acute care experience preferred.
  • Previous nursing leadership or management experience is a plus.
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to work effectively with residents, families, CNAs, RNs, physicians, and other healthcare professionals.
  • Compassionate, dependable, and committed to providing excellent resident care.

Schedule

Night Shift: 7:00 PM–7:00 AM
