Core Responsibilities
- Provide skilled nursing care to hospice patients in accordance with the established plan of care, under the direction of a Registered Nurse or licensed provider
- Perform clinical interventions and treatments within LPN scope of practice, including medication administration as prescribed
- Observe, document, and report patient condition, symptoms, and responses to care
- Support comfort measures, activities of daily living, and symptom control
- Communicate effectively with patients, families, and members of the interdisciplinary Hospice team
- Participate in care coordination and contribute to the modification of care plans as directed
Qualifications
- Education: Graduate of an accredited Licensed Practical Nurse program
- Experience: Open to New Grads; Hospice, home health, long-term care, or acute care experience helpful
- Licenses & Certifications: Current Licensed Practical Nurse (LPN) license in the state of practice; Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
- Additional Requirements: Valid driver’s license with proof of auto insurance; Ability to travel to patient homes within the service area
