About the role

Adoration Health is seeking a Hospice LPN to join their team in Altoona, PA. The position offers 1:1 care, flexible scheduling, and opportunities for professional growth.

Responsibilities

  • Provide hands-on care to patients nearing the end of life, adhering to state nursing rules.
  • Absess the patient's needs and follow the care plan.
  • Keep clear and timely records of the care provided.
  • Teach and support patients and their families about the illness and care plan.
  • Communicate with the hospice team to coordinate care.
  • Manage pain and symptoms, evaluating treatment effectiveness.
  • Take part in training and quality improvement activities.

Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience.
  • Community health/hospice or medical/surgical experience preferred.
  • Currently licensed as an LPN/LVN in the State.
  • Comply with accepted professional standards and practice.
  • Must be a licensed driver with an insured vehicle in good condition.
  • Possess and maintain current CPR certification.
  • Understand the hospice concept and the needs of the terminally ill.
  • Use knowledge and resources to make logical decisions and solve problems.
  • Show good verbal and written communication and organizational skills.

Qualifications

  • Currently licensed as an LPN/LVN in the State.
  • Comply with accepted professional standards and practice.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possess and maintain current CPR certification.
  • Understand the hospice concept and the needs of the terminally ill.
  • Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
