LPN / Licensed Practical Nurse - Hospice

About the Role

Hospice of North Alabama is seeking a passionate, dedicated Hospice LPN to join our team in Parkersburg, WV. Our hospice LPNs provide expert, patient-centered care at a critical time in people’s lives in Wood, Pleasants, Ritchie, Wirt, Calhoun, and Roane counties.

Schedule

Monday through Friday, 8am–5pm.

Responsibilities

• Provide hands-on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient’s needs.
• Maintain clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Coordinate care by communicating with the hospice team (aides, chaplains, social workers).
• Manage pain and symptoms, evaluating the effectiveness of treatments.
• Participate in training and quality improvement activities to grow professionally and improve care.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; community health/hospice or medical/surgical experience preferred.
• Current LPN/LVN license in the State.
• Compliance with accepted professional standards and practice.
• Licensed driver with an insured automobile in good working order.
• Current CPR certification.
• Understanding of hospice philosophy and needs of the terminally ill.
• Ability to make logical decisions and solve problems using knowledge and resources.
• Strong verbal, written, and organizational skills.
