Join The Care Team as a part-time Hospice LPN and make a meaningful impact by providing compassionate, hands-on care and support to patients and families during life’s most important moments.
Responsibilities
- Provide nursing care as delegated by the RN
- Maintain patient status and report changes
- Support comfort and symptom management
- Document care provided in the EMR
- Collaborate with the interdisciplinary team
Qualifications
- Active Licensed Practical Nurse (LPN) license
- Hospice or long-term care experience preferred
Field Role Requirements
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
