The Care Team Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join their growing team. This position offers hands-on patient care, support for individuals and families during the hospice journey, and the opportunity to work alongside an experienced interdisciplinary team.

Responsibilities

• Deliver skilled nursing care
• Monitor patient needs
• Ensure patients receive comfort, dignity, and support
• Build meaningful relationships with patients and families

Requirements

• Current LPN license in good standing
• Experience in hospice care preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Bachelor's degree in Nursing preferred
• Current CPR certification
