About the role

Actify Home Care has active LPN home health cases ready for assignment across its expanding territory, including South Palm Beach County: Boca Raton, Delray Beach, Boynton Beach, Lantana, and surrounding areas.

Responsibilities

• Provide high-quality, one-on-one therapy to geriatric patients in their homes.
• Implement and follow treatment plans established by the supervising RN.
• Chart patient progress and treatment responses accurately and efficiently.
• Collaborate with the supervising RN to coordinate and communicate patient status.

Requirements

• Active Florida Registered Licensed Practical Nurse (LPN) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.
