Licensed Practical Nurse LPN (Home Care) Central Palm Beach County

About the role

Actify Home Care is looking for LPNs to work in the Central Palm Beach County area, including West Palm Beach, Wellington, Royal Palm Beach, Riviera Beach, Lake Worth and surrounding areas.

Responsibilities

• Provide high-quality, one-on-one therapy to geriatric patients in their homes.
• Implement and follow treatment plans established by the supervising RN.
• Chart patient progress and treatment responses accurately and efficiently.
• Collaborate with the supervising RN to coordinate and communicate patient status.

Requirements

• Active Florida Registered Licensed Practical Nurse (LPN) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.

Qualifications

• Kinnser/WellSky experience is a major plus!

• Effective communication skills.
• Ability to work independently and as part of a team.
• Strong organizational and documentation skills.

Pay

Hi-Tech / Complex Care Visits: $35.00
Routine Follow-up Visits: $30.00
