Licensed Practical Nurse LPN (Home Health) South Broward County

About the role

Actify Home Care is seeking LPNs for home health assignments across its expanding territory in South Broward County, including Hollywood, Pembroke Pines, Miramar, Davie, Weston, Hallandale Beach, and surrounding areas.

Responsibilities

• Provide high-quality, one-on-one therapy to geriatric patients in their homes.
• Implement and follow treatment plans established by the supervising RN.
• Chart patient progress and treatment responses accurately and efficiently.
• Collaborate with the supervising RN to coordinate and communicate patient status.

Requirements

• Active Florida Registered Licensed Practical Nurse (LPN) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.

Pay

• Hi-Tech / Complex Care Visits: $35.00
