Licensed Practical Nurse (LPN) │Full-Time

About the role

Provides essential nursing care by assessing residents, documenting findings, and communicating with the interdisciplinary team. Implements care plans, administers medications and treatments safely, and updates care strategies as needed. Ensures timely execution of physician orders and adheres to safety protocols, including IV procedures for certified staff. Maintains composure during crises and prioritizes resident safety and well-being.

Responsibilities

• Assess and monitor residents' conditions, reporting significant changes to the team.
• Participate in care planning and update plans based on interventions.
• Administer medications, treatments, and IV fluids following certification standards.
• Accurately document assessments, interventions, and resident responses.
• Communicate effectively with residents, families, and staff using SBARR and other tools.
• Respond to call lights, assist with ADLs, and monitor residents' behavioral and physical status.
• Engage in ongoing nursing education to stay current with best practices.
• Identify issues and contribute to problem-solving through quality improvement processes.

Qualifications

• Licensed Practical Nurse (LPN) licensure in the relevant state.
• Current CPR, AED, and IV certification (or willing to obtain).
• Ability to stand, walk, lift, and transfer residents, including those over 200 lbs, with proper techniques and assistance.
• Manual dexterity and visual acuity, including color recognition.
• Experience in long-term care preferred.
