Licensed Practical Nurse (LPN) - Evening

About the role

As a Licensed Practical Nurse (LPN), you will work under the direction of licensed nursing personnel to deliver direct and indirect resident care. You will play a vital role in ensuring the comfort, safety, and well-being of our residents while adhering to individualized care plans. This position requires knowledge of growth and development principles across the lifespan and the ability to address age-specific needs.

Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including hygiene, grooming, dressing, toileting, skin care, and mobility, as outlined in care plans.
• Maintain a safe and clean environment by promptly answering call lights, ensuring proper use of equipment, preventing falls, and keeping units tidy.
• Support residents' nutritional needs by delivering meals, assisting with feeding, and honoring dietary preferences.
• Provide rehabilitative care, such as range-of-motion exercises, ambulation assistance, transfers, and proper body alignment.
• Report changes in residents' physical or emotional conditions to the Charge Nurse promptly.
• Document all care provided according to protocol.

Requirements

• Current NYS LPN Licensure
• High School Diploma or GED equivalent.
• BLS certification (AHA/RQI specifically)
• 1 year experience as LPN
• Compassionate demeanor with excellent communication skills.

Work Environment

This position involves occupational exposure to bloodborne pathogens and requires adherence to safety protocols. Tasks may involve contact with blood, body fluids, or tissues.
