Licensed Practical Nurse LPN

The Center for Rehabilitation & Nursing at Washington Township in Mantua Township, New Jersey is seeking a compassionate and dedicated Licensed Practical Nurse (LPN).

Responsibilities

• Provide direct patient care under the supervision of a registered nurse, including administering medications, monitoring vital signs, and performing treatments as prescribed by physicians
• Develop and implement individualized care plans in collaboration with interdisciplinary team members
• Conduct patient assessments, identify areas of concern, and report findings to registered nurses and other healthcare professionals
• Maintain accurate and detailed records of patient care, treatments, and progress
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Maintain infection control and safety standards in accordance with facility policies and regulations

Requirements

• Current LPN licensure in the state of New Jersey
• High school diploma or equivalent required; degree or diploma in nursing preferred
• Minimum 0 years of experience; new graduates welcome to apply
• Ability to work effectively in a fast-paced environment with multiple priorities
• Excellent communication and interpersonal skills
• Strong observation, assessment, and problem-solving skills
• Ability to maintain confidentiality and handle sensitive information
