About the Role

As a Licensed Practical Nurse in Home Health at Caretenders, a part of LHC Group and the Optum family of businesses, you will provide quality patient care and perform technical skilled care in the patient's home. This role involves creating meaningful connections with patients, their families, and the community while adhering to a culture of Caring, Connecting, and Growing together.

Responsibilities

  • Provide high-quality clinical services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with the healthcare team, following all applicable infection control procedures.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency's electronic visit verification program.
  • Actively participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, clinicians, agency management, and community case managers.
  • Function effectively in any home situation regardless of the client's age, race, creed, color, sex, disability, or financial condition.

Requirements

  • Current and unrestricted LPN licensure in the state of practice.
  • Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Preferred Qualifications

  • Current CPR certification or ability to complete within 90 days of hire.
  • Home care experience.
  • Ability to manage multiple tasks simultaneously.
  • Able to work independently.
  • Good communication, writing, and organizational skills.
  • Ability to work flexible hours as required to meet identified client needs.

Pay

Annual total cash target pay range: $46,564 - $69,846.

  • Hourly rate: $22.39 - $33.58.
  • Per visit point rate: $26.86 - $40.30.

Annual total cash compensation assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors, including local labor markets, education, and work experience. Compensation may increase over time based on productivity and performance in the role.

This role receives two types of compensation depending on the work being performed:

  • For visits: Paid per visit point rate, calculated by multiplying the per visit point rate by the productivity points accrued for various types of visits. Each type of visit is assigned a certain number of productivity points inclusive of "direct" and "indirect" patient care activities. The number of visits performed each week will vary based on individual productivity targets.
  • For non-visit activities (e.g., orientation): Paid hourly rate.
