Licensed Practical Nurse - Family Medicine

About the role

The LPG Family Medicine practice located on Plantation Road is seeking an experienced LPN to join their team. This LPN will conduct phone triage, room patients, administer medications, and assist providers in the office. Responsibilities include providing an excellent customer experience while ensuring patient care is delivered safely, effectively, and efficiently.

This position involves collecting patient health data and safely delivering and documenting selected acts such as administration of treatments and medications. The LPN will collect data either directly or telephonically in a systematic and ongoing manner, based on patient response to treatments, medications, and changes in condition. The role includes implementing patient care and education following providers' orders or under the guidance of a registered nurse, while incorporating knowledge of patient's age and cultural diversity.

The position ensures care is delivered safely and meets the requirements of the Florida Nurse Practice Act, licensure, regulatory agencies, and Lee Health policies and procedures. The LPN is responsible for professional communication, patient education, and collaboration with other healthcare team members to achieve desired patient outcomes.

Responsibilities

• Conduct phone triage and room patients.
• Administer medications and assist providers in the office.
• Collect and document patient health data systematically and continuously.
• Implement patient care and education following providers' orders or under the guidance of a registered nurse.
• Ensure safe delivery of care in compliance with the Florida Nurse Practice Act, licensure, and regulatory requirements.
• Collaborate with healthcare team members to achieve desired patient outcomes.
• Provide professional communication and patient education.

Requirements

• High School diploma or equivalent required.
• Graduate of an accredited Practical/Vocational Nursing program required.
• Physician Practice/Outpatient experience preferred.
• BLS (American Heart Association/Basic Life Support) certification required.
• 30-hour IV course may be required based on specialty.
• Currently licensed as an LPN in the state of Florida.
• Above average written and verbal communication skills.
• Adaptable and participative with technological changes.
• Proficient use of Electronic Health Record (EHR) preferred.
• Early/late hours and/or weekends may be required.

Pay

$22.34 - $29.04 per hour (minimum to midpoint pay rate).

Schedule

Full-time, Shift 1: 8:30 AM to 5:00 PM.
