About the role
As part of a multidisciplinary team, you will deliver patient-centered nursing and rehabilitation services to patients. You will support the Unit Manager to maintain excellent clinical standards and patient experience while providing the highest level of service to all patients, families, and visitors to ensure satisfaction.
Responsibilities
- Supervises and makes assignments of CNAs on the unit.
- Follows the nursing care plan, utilizes nursing diagnosis for patients in cooperation with the supervisor and nursing personnel on the unit.
- Follows the plan of care for residents.
- Consistently, accurately, and independently prioritizes and completes routine assignments within guidelines and schedules.
- Recognizes psychosocial needs, their possible effects on behavior, and intervenes to provide emotional support.
- Maintains accurate and informative records on all patients in the unit.
- Identifies and reports all real or potential problems regarding the development or administration of patient care.
- Records, reports, and interprets patient responses to treatment and care to appropriate members of the interdisciplinary team.
- Identifies any changes in patient conditions/needs and reports to the Charge Nurse.
- Demonstrates a team approach to problem-solving through frequent communication and collaboration on complex issues to provide appropriate care.
- Maintains all equipment as per facility policy and uses equipment consistent with equipment specifications.
- Participates and represents Harborage and Nursing staff on committees, as requested by supervisors.
- Identifies patient needs and treatment opportunities, providing services to meet those needs on an ongoing basis.
- Maintains professional competence and keeps current with Occupational Health trends through participation in continuing education, in-service programs, professional staff sessions, and training of nursing personnel.
- Fosters good relationships and communications with all team members, patients, family/significant others, and visitors.
- Assists with new team member orientation and provides key experiences to prepare team members for independent performance of essential job functions.
- Reports problems to the Manager promptly.
- Performs other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Requirements
- Completion of an approved LPN program.
- Current with all Continuing Education (CE) requirements.
- Ability to perform clinical care professionally.
- Serve as a patient advocate and role model to other team members.
- Deliver patient care with minimal disruptions and obstacles.
- Ability to effectively work on a multidisciplinary team.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
- Endurance to stand and walk for extended periods of time.
- Ability to lift, push, and pull a minimum of 50 pounds.
Qualifications
- Licensed Practical Nurse (LPN).
- BLS AHA Basic Health Care Life Support HCP Certification within 60 days of entering position.
Preferred:
- Long-term care facility experience.
- Prior work experience with geriatric and disabled populations.
- Bi-lingual (Spanish).
- IV Certification.
Pay
Minimum rate of $32.97 hourly.
