Licensed Practical Nurse - Community Internal Medicine

About the role

Provides patient-centered nursing care under supervision, collaborating with providers and the healthcare team in a fast-paced, team-oriented primary care environment to build long-term patient relationships.

Responsibilities

• Administer vaccines, medications, and treatments
• Manage patient communications via various platforms
• Support preventive and chronic care
• Ensure timely, accurate care delivery
• Participate in quality improvement initiatives
• Provide virtual support as needed
• Float to other sites and work some Saturdays

Requirements

• Graduation from an accredited practical nursing program or passing the LPN boards
• Current LPN license
• Maintain BLS certification
• Excellent communication and computer skills
• Flexibility and adaptability to unpredictable situations

Qualifications

Preferred experience of at least one year in a relevant setting.
