What You'll Do:
- Plan, coordinate, and implement personalized care plans
- Administer medications and treatments in accordance with physician orders
- Maintain accurate documentation and follow infection control procedures
- Work collaboratively across teams to ensure a safe, supportive environment
- Respond to resident needs with compassion and professionalism
What You'll Need:
- Licensed Practical Nurse (LPN) with valid Illinois license (required)
- One to two years of experience in a long-term care environment (preferred)
- Graduate Registered Nurse (GRN) will be considered if licensure is received within 45 days of employment
- Strong interpersonal and communication skills, both verbal and written
