About the role

The Licensed Practical Nurse at The MENTOR Network is responsible for providing medical treatment and personal care services to individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges. This role involves traveling between multiple sites and may require availability after hours for consolation.

Responsibilities

  • Provides basic care and medical treatment to individuals receiving services.
  • Completes health assessments and provides input of health diagnoses to RN or physician for the development of nursing care plans.
  • Follows-up on all physician orders.
  • Makes sure individual’s medical/dental appointments are monitored and recorded.
  • Monitors and records medications in a timely manner.
  • Acts as a liaison to pharmacy and reviews medication error forms, analyzing errors and making recommendations for procedural changes.
  • Administers medication as required and oversees staff administering medication.
  • Conducts quality assurance checks on health care records.
  • Provides emergency coverage and/or care for individuals receiving services.
  • Maintains confidentiality, respects human rights, and practices universal precautions in accordance with Company policy.
  • Partners with external agencies to coordinate services and ensure consistent treatment.
  • Administers, tracks, and maintains logs for staff members’ and individuals’ testing.
  • Trains staff members and individuals receiving services in various topics.
  • Coordinates, organizes, and assists with household activities and transports individuals to planned and necessary activities, appointments.
  • Supports staff at all locations during working hours and may be required to be available after hours for consolation.
  • Participates in on-call system as needed.

Requirements

  • Associate’s Degree in Nursing or related field, or a graduate of an accredited LPN program.
  • One year experience working with population served in a nursing capacity.
  • Current state LPN License.
  • Current driver's license, car registration, and auto insurance if driving on behalf of the Company or providing transportation to individuals receiving services.
  • Certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
  • Physical requirements: heavy work, exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.

Skills

  • Strong communication skills.
  • Ability to work independently and as part of a team.
  • Knowledge of universal precautions and infection control.
  • Proficiency in Microsoft Office Suite.

Pay

$X per hour, depending on experience and qualifications.
