Licensed Practical Nurse
About the role
D&S Community Services is a mission-driven organization dedicated to supporting individuals with various disabilities through community-based services. The role of Licensed Practical Nurse/Licensed Vocational Nurse involves providing comprehensive nursing care, administering medications, conducting assessments, and coordinating with other healthcare professionals.
Responsibilities
- Maintain the health conditions of individuals served and ensure quality services are provided.
- Administer medications and document accordingly.
- Oversee and monitor staff members administering medications.
- Provide prescribed medical treatment based on physician’s orders.
- Implement health care services as outlined in each Individual Service Plan (ISP).
- Respond to changes in the condition of individuals served.
- Train staff members on universal precautions, infection control, and medication administration.
- Serve as a liaison and advocate with other medical and professional service providers.
- Coordinate services with external agencies to ensure consistent treatment.
- Train individuals served on self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
Requirements
- Associate’s Degree in Nursing or related field, or state certificate.
- One year of experience working with the population served in a nursing capacity.
- Current state LPN or LVN licensure for the state of residence.
- Current driver's license, car registration, and auto insurance.
- Other certifications as required by state licensing regulations, such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
