Responsibilities
- Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.
- Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).
- Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.
- Offer education and emotional support to patients and their families/caregivers based on individual needs.
- Supervise LPNs/LVNs, home health aides, and other support staff.
- Participate in on-call rotations, admissions, or urgent response as needed (common in hospice roles).
- Cookordination care activities, including visit scheduling and documentation to ensure compliance with regulations.
- Maintain accurate clinical records and ensure care aligns with agency policies, professional standards, and regulatory requirements.
Qualifications
- One year of clinical experience and one year of home health experience.
- Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records.
- Required Certifications And Licensures:
- Graduate of an accredited practical or vocational nursing program.
- Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver who can travel to all business locations.
