About the Role

The Licensed Practical Nurse provides direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations governing the community. This role ensures the highest degree of quality care is maintained at all times under the direction of the RCC Services or Nurse Supervisor.

Responsibilities

  • Comply with the community’s personnel, safety, and corporate policies and procedures, as well as nursing department policies.
  • Report allegations of resident abuse or neglect immediately to the Executive Director, RCC, or Unit Manager.
  • Treat residents, family members, visitors, and team members with dignity and respect while maintaining confidentiality.
  • Assist in the admission, transfer, and discharge of residents.
  • Provide daily hygiene care, including bathing, oral care, hair and nail care, and assistance with bowel and bladder functions.
  • Keep residents dry by changing clothing and linen when soiled or wet.
  • Assist in transporting residents to appointments, activities, and social programs.
  • Lift, turn, move, position, and transport residents using proper techniques and equipment.
  • Report and document changes in a resident’s condition or behavior to the Charge Nurse.
  • Provide care for dying residents and perform post-mortem care.
  • Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, and blood draws as required.
  • Perform lab tests, blood pressure monitoring, and other routine tasks per the resident care plan.
  • Complete accident/incident reports, medical forms, evaluations, and charting in an informative and descriptive manner.
  • Ensure accurate recordkeeping for resident admissions, transfers, and discharges.
  • Report discrepancies in physician’s orders, diet changes, or charting errors to the Nurse Supervisor.
  • Develop and maintain good working relationships with inter-departmental personnel to meet resident needs.
  • Review complaints and grievances from assigned personnel and report them to the Nurse Supervisor.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals per community policies.
  • Monitor assigned personnel to ensure compliance with safety regulations for equipment and supplies.
  • Recommend equipment and supply needs to the Nurse Supervisor.
  • Review care plans daily to ensure appropriate care is provided.
  • Oversee resident needs, staff, and operations as needed.

Requirements

  • Valid professional LPN license in good standing with the state.
  • Basic computer skills; ability to read, write, speak, and understand English.
  • Ability to work beyond normal hours, weekends, and holidays as necessary.
  • Ability to assist in resident evacuations during emergencies.
  • Ability to perform essential job functions with or without reasonable accommodation.

Physical Requirements

  • Regular worksite attendance.
  • Frequent lifting of up to 25 lbs; occasional lifting of 26–50 lbs.
  • Constant carrying of up to 10 lbs; occasional carrying of 11–25 lbs.
  • Frequent reaching above shoulder height; constant reaching at or below shoulder height.
  • Constant pushing, pulling, grasping, handling, and fingering; frequent torquing.
  • Frequent twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing.
  • Ability to operate an automobile and assist residents with mobility devices.
  • Ability to work with ill, disabled, elderly, emotionally upset, or hostile individuals.
  • Sufficient vision and hearing (or use of prosthetics) to perform job requirements.
  • Use of a transfer/gait belt or assistance from another employee when lifting or carrying objects over 25 lbs.
