Licensed Practical Nurse

Role Overview

This position involves providing patient-centered outpatient nursing care in a primary care setting, supporting diverse services including family practice, OB/GYN, pediatrics, behavioral health, and specialty areas.

Key Responsibilities

• Perform therapeutic nursing interventions such as daily patient care, tube feeds, and dressing changes under RN supervision.
• Administer medications following completion of required training.
• Accurately document care using the EPIC system.
• Collaborate within an interdisciplinary team, including nurses, social workers, care managers, and other healthcare professionals.
• Ensure quality care aligned with standards and practices.

Qualifications & Skills

• Graduate from an accredited LPN program with current New York licensure.
• Maintain BLS/CPR certification.
• Participate in ongoing education and training.
• Ability to lift up to 50 pounds, stand, walk, sit, bend, and reach as needed.
