Responsibilities
Enjoy a consistent Monday–Friday schedule with no evenings, no weekends, and no on-call responsibilities.
Predictable, Local Travel. This role is shared between two nearby communities (just minutes apart), with a consistent rotating schedule: Week 1: 3 days at Location A, 2 days at Location B (Crystal)
Week 2: 2 days at Location A, 3 days at Location B (Brooklyn park)

Position Overview

As a Licensed Practical Nurse (LPN), you will play a key role in delivering high-quality, resident-centered care across two Assisted Living and Memory Care communities.
You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure safe, compassionate, and personalized care.

Key Responsibilities

  • Provide direct nursing care, including medication administration, treatments, and assessments
  • Monitor resident health and communicate changes to the care team
  • Aid in developing and updating individualized care plans
  • Support residents with memory loss through patience, empathy, and engagement
  • Build strong relationships with residents, families, and team members
  • Maintain accurate and timely clinical documentation
  • Mentor and support caregivers and new staff
  • Ensure compliance with state regulations and company policies

Qualifications

  • Graduate of an accredited practical nursing program
  • Active Minnesota LPN license in good standing
  • Preferred (not required): Assisted Living, Memory Care, or Long-Term Care experience
