About the Role

We are looking for a proactive employee to provide clinical and therapeutic services to children with a range of mental health concerns and/or intellectual and developmental disabilities, in the Critical Time Transition Program.

Responsibilities

  • Complete initial evaluations and re-evaluations for assigned individuals on CTI caseloads
  • Use professional judgement and training to develop goals for assigned individuals
  • Use best practices in performing/implementing counseling services
  • Maintain appropriate contact with assigned individuals, their families, guardians, and care managers
  • Perform home evaluations, sessions, or training, as needed
  • Maintain full caseload; schedule make-up visits for cancellations in order to provide adequate support during time of transition for participant
  • Utilize proactive approaches to avert and/or de-escalate crisis situations
  • Participate in the staff on-call rotation, providing after-hours support and interventions via phone, as needed
  • Provide information and resources regarding diagnoses, disabilities, and treatment
  • Conduct self in a professional manner
  • Attend staff meetings and other meetings as directed by supervisor
  • Participate in in-service programs
  • Participate in Agency-wide events, advocacy, and/or, community education
  • Provide support to other Agency programs, as needed
  • Provide person-centered, quality, services
  • Demonstrate effective time management skills
  • Observe confidentiality and safeguards regarding all participant related information
  • Follow Agency standards and procedures
  • Maintain compliance with applicable program rules and regulations
  • Contribute to department and Agency quality improvement initiatives
  • Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies
  • Perform Other Duties as Assigned

Education & Experience

  • Master's degree in social work or Mental Health Counseling, required
  • Provisionally licensed in NYS (LMSW, LCSW, or LMHC) and working to obtain licensure, required
  • Currently licensed, preferred
  • One year of experience working with children and adults with developmental disabilities and/or mental health diagnoses, preferred

Reports to: Director, CTTP | Location: Basile Rowe, East Syracuse, NY 13057 | Pay Rate: $70,720 - $74,880 | Schedule: Monday - Friday: 8 a.m. - 4 p.m.
