Lead Therapist

About the role

Continuum Therapy Partners brings a therapist-led approach to scale, founded by industry veterans with twenty years of experience striving for a more responsive therapy environment. We are seeking Speech-Language Pathologists to join our team of 285 locations across 16 states, where we prioritize and advocate for the professional perspective of our clinicians.

Responsibilities

• Deliver skilled therapy services within the scope of licensure (PT, PTA, OT, OTA, or SLP) in accordance with physician orders and individualized plans of care.
• Assist the Director of Rehabilitation (DOR) or Regional Vice President (RVP) with daily therapy operations, acting as the point of contact in their absence.
• Collaborate on clinical program development to meet patient needs and facility goals.
• Foster positive working relationships with facility nursing, administration, and interdisciplinary team (IDT) members.

Requirements

• Graduate of an accredited program in Physical Therapy, Physical Therapist Assistant, Occupational Therapy, Occupational Therapy Assistant, or Speech-Language Pathology.
• Licensed and in good standing in the state of practice.
• Minimum of 2 years of clinical experience in a skilled nursing or long-term care setting.

Schedule

• Full-time or Part-Time, daytime.
• Primary location: Mineral Point Health Services in Mineral Point, Wisconsin with occasional coverage in Lancaster, WI at Lancaster Health Services.
