Lead Street Outreach Case Manager

About the role

The Lead Street Outreach Case Manager is responsible for providing direct services to individuals experiencing homelessness, collaborating with community partners, and ensuring that clients are connected to necessary resources.

Responsibilities

• Conduct outreach in various neighborhoods to identify and engage individuals experiencing homelessness
• Assess client needs and provide initial case management services
• Coordinate with social service agencies to secure housing, healthcare, and other essential services
• Document interactions and progress with clients in a case management system
• Maintain regular communication with clients to ensure their ongoing support and engagement
• Participate in team meetings and trainings to stay updated on best practices and new initiatives

Requirements

• Bachelor’s degree in Social Work, Public Health, or related field
• At least 2 years of experience in direct service work with homeless populations
• Proven ability to build relationships and collaborate with diverse stakeholders
• Strong organizational skills and proficiency in Microsoft Office applications
• Valid driver’s license and access to reliable transportation

Qualifications

• Experience working with individuals from marginalized communities
• Knowledge of local resources and services for homeless individuals
• Ability to work flexible hours including evenings and weekends

Skills

• Excellent verbal and written communication skills
• Proficiency in computer software such as Microsoft Office
• Ability to work independently and as part of a team
