About the role

The Street Outreach Case Manager is responsible for providing direct services to individuals experiencing homelessness in the community.

Responsibilities

  • Conduct outreach to identify and engage individuals experiencing homelessness
  • Assess needs and provide case management services
  • Coordinate with other service providers to ensure comprehensive support
  • Maintain accurate records and reports
  • Participate in team meetings and training sessions

Requirements

  • Bachelor’s degree in social work, counseling, or a related field
  • At least 2 years of relevant experience in homelessness services
  • Valid driver’s license and access to reliable transportation
  • Ability to work flexible hours including evenings and weekends

Qualifications

  • Strong interpersonal and communication skills
  • Proficiency in Microsoft Office Suite
  • Ability to work independently and as part of a team
