About the role
The Street Outreach Case Manager is responsible for providing direct services to individuals experiencing homelessness in the community.
Responsibilities
- Conduct outreach to identify and engage individuals experiencing homelessness
- Assess needs and provide case management services
- Coordinate with other service providers to ensure comprehensive support
- Maintain accurate records and reports
- Participate in team meetings and training sessions
Requirements
- Bachelor’s degree in social work, counseling, or a related field
- At least 2 years of relevant experience in homelessness services
- Valid driver’s license and access to reliable transportation
- Ability to work flexible hours including evenings and weekends
Qualifications
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
- Ability to work independently and as part of a team
