Lead Social Worker
Job Summary
The Lead Social Worker at HCA Healthcare Trident Hospital is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
Other Duties & Responsibilities
- Acts as a liaison between the patient and community resources/post-acute care providers.
- Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care.
- Facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.
- Provides orientation and mentorship to new and existing staff, collaborates with the Director of Case Management in the hiring and interview process for new professional staff and manages a limited case load in addition to providing guidance and consultation to social work staff.
Qualifications
- A Master’s Degree in Social Work is required.
- Licensure as a LMSW or a LCSW is required.
- At least 2 years of experience as a Social Worker is required.
- Prior experience in the acute care setting strongly preferred.
