About the role
Recognized leadership abilities and strong clinical background, who will provide assistance to the department manager in the continuing operation of the department, and facilitate the provision of professional services to meet the psychosocial needs of the patient through psychosocial assessments, brief treatment/counseling, crisis interventions, case consultation, patient education, resource management, and assistance with discharge planning.
Responsibilities
- Provide leadership to other social workers in the department with guidance and oversight.
- Serve as an integral part of the interdisciplinary team, providing mental health expertise and community outreach/networking with community programs as needed.
- Work closely with social service staff to coordinate services for patients according to policies, procedures, and guidelines.
- Provide care and/or service to the critically ill, rehabilitating newborn, and the geriatric patient population in a manner that demonstrates an understanding of the functional and/or developmental age of the individual served.
- Demonstrate behavior consistent with the Core Values and support the organization’s strategic plan and Performance Improvement Plan (PIP).
- Consistently apply infection control policies/practices, standard precautions, and disease-specific isolation.
- Meet population/age-specific competencies per unit-specific addendum.
- Attend department-specific education/training, inservices, and staff meetings, including mandatory activities.
- Submit all required paperwork on time and verify attendance at staff meetings or reading of minutes.
- Participate in and coordinate administrative projects; provide administrative support to the Department Director.
- Utilize risk management strategies to minimize organizational liability.
- Demonstrate the ability to apply the change process in support of organizational imperatives.
- Assist in maintaining department readiness for all regulatory requirements.
- Serve as a role model for team building behaviors and a resource during crisis or disaster situations.
- Provide assistance in the development of department policies and procedures.
- Assist with assignments/schedules at the direction of the Director.
- Demonstrate ability to provide competent and age-appropriate clinical care.
- Maintain appropriate knowledge and skills to serve as a resource to staff for patient care.
- Provide support for case management through knowledge of basic utilization review concepts, discharge planning, and general payor requirements.
- Utilize a systematic approach, including physiological, psychological, spiritual, environmental, and age-related factors when assessing patient care needs.
- Provide safe and competent care in emergency situations.
- Provide a variety of therapeutic and interventional modalities to meet patients’ psychosocial needs.
- Provide timely and appropriate assessments and interventions for patients.
- Advocate for patients’ rights.
- Maintain knowledge of community resources and demonstrate ability to mobilize them on patients’ behalf.
- Maintain appropriate documentation and statistical records.
Education/Training/Experience
- Master’s degree from an accredited school of Social Work or Master’s Degree from an accredited psychology program.
- Three years of acute care hospital experience.
- Leadership experience preferred.
Licenses/Certifications
- Licensed as a Masters prepared social worker or marriage and family therapist.
- BLS and LCSW license required.
