Lead Social Worker

About the Role

Plan, direct, define, and coordinate individualized and group therapeutic support and services for patients with mental health, behavioral health, and substance use issues.

Responsibilities

• Conduct social history assessments of patients through observation and patient/family interviews, gathering information on needs, skills, interests, and limitations to develop and implement treatment plans.
• Coordinate and plan programs and activities to meet patients' social and emotional needs.
• Promote recovery, symptom reduction, and achievement of the highest level of social functioning.
• Provide crisis management and family sessions to help families understand the implications and complexities of life situations and their impact on lifestyle.
• Assist in developing effective treatment plans that apply therapeutic principles while ensuring patient safety.
• Participate in multidisciplinary treatment team meetings to discuss patient assessments, treatment plans, and ongoing discharge plans.
• Communicate with or aid family members in understanding and supporting the patient.
• Manage a caseload and assume responsibility for patient chart documentation.
• Facilitate education, process groups, support groups, and referrals as requested.
• Demonstrate a positive, empathetic, and professional attitude toward customers at all times. Address and resolve complaints when patient needs are not met.
• Recognize that patient safety is a top priority.
• Perform other functions and tasks as assigned.

Requirements

• Bachelor’s degree in Social Work from an accredited college or university (new graduates accepted).
• One or more years of experience working with the patient population preferred.

Qualifications

• Eligible for state licensure as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), or Registered Addiction Counselor (RAC), as required by the state.
• CPR and de-escalation/restraint certification required (training available upon hire and offered by the facility).
• First aid certification may be required based on state or facility.
