Responsibilities
Lead, Registered Nurse (RN)
- Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic, and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
- Maintain current knowledge, certification, and licensure.
- Demonstrate the skills and judgment necessary to implement the medical plan of care, nursing interventions, and procedures necessary for patient care.
- Perform delegated nursing functions using established procedures, policies, guidelines, and standards.
- Assist in data collection on the EMR and document pertinent observations and interventions on appropriate records.
- Contribute to the development of the patient/significant other plan of care based on identified patient problems/needs.
- Report needs of the patient and observations regarding condition to the provider.
- Carry out physician orders accurately, directly, and indirectly.
- Perform selected tasks related to patient care and the patient’s environment in a safe manner.
- Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
- Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within the medical record, client, and healthcare team feedback.
- Assure effective communication exists with the patient/significant other as reflected in the plan of care.
- Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
- Assist patients, family members, visitors, or employees with concern and empathy; respect their confidentiality and privacy and communicate courteously and professionally.
- Answer and refer telephone calls or other inquiries to ensure accurate and timely communications.
- Assist in the orientation/training of new team members and function as a role model for current/new staff.
- Consistently work in a positive and cooperative manner with fellow team members, other team leads, and nursing supervisors.
- Take direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
- Demonstrate flexibility to perform duties wherever volume deems it necessary within the clinic.
- Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns.
- Ensure compliance with regulatory standards.
- Develop schedules for team members.
Lead, Licensed Practical Nurse (LPN)
- Perform medication reconciliation with patients.
- Advise and schedule appropriate follow-up appointments.
- Carry out physician orders accurately, directly, and indirectly.
- Request appropriate medical records.
- Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical records, client, and healthcare team feedback.
- Effectively communicate patients’ changing needs and plans of care among staff members.
- Report patient needs/programs and observations regarding patient’s condition to the physician.
- Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
- Participate in staff development programs.
- Attend scheduled staff meetings and be responsible for information discussed.
- Assist patients, family members, or other clients with concern and empathy; respect their confidentiality and privacy and communicate courteously and professionally.
- Answer and refer telephone calls or other inquiries to ensure accurate and timely communications.
- Assist in the orientation/training/mentoring of new team members.
- Consistently work in a positive and cooperative manner with fellow team members, other team leads, and nursing supervisors.
- Assist other team members in the performance of their assignments.
- Take direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
- Demonstrate flexibility to perform duties wherever volume deems it necessary within the clinic.
- Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns.
- Consistently evaluate work and determine if further steps are needed to meet client expectations.
- Ensure compliance with regulatory standards.
- Demonstrate ability to respond to changing situations in a flexible manner to meet current needs, such as reprioritizing work as necessary.
- Minimize non-productive time and fill slow periods with activities that enable preparation to meet future needs of the clinic (education, organizing, housekeeping, assisting others).
- Organize job functions and work area to effectively complete varied assignments within established time frames.
- Demonstrate ability to take initiative to make decisions/choices without direct supervision.
Requirements
Lead, Registered Nurse (RN)
- Graduate of an accredited school of nursing.
- One year of relevant work experience.
- Current, unrestricted MO RN license.
- Current Basic Life Support (BLS) required, must obtain within first 90 days of employment.
- IV certified.
Lead, Licensed Practical Nurse (LPN)
- Graduation from LPN program and current, unrestricted MO LPN license.
- Minimum of two years’ clinical experience.
- Current Basic Life Support (BLS) required, must obtain within first 90 days of employment.
Preferred Qualifications
- Experience in ambulatory clinical setting preferred.
- Experience with electronic medical records preferred.
