About the Role

This position provides in-office case management services to families within the emergency shelter program. The Lead Case Manager evaluates and assesses immediate needs, oversees accurate data quality of all files upon client entry, connects families with services and resources, and is responsible for outcomes reports prior to client exit from the Emergency Shelter and Kitchenette housing program.

Responsibilities

  • Provide weekly case-management services to assigned clients, including developing client support plans, goals, and resources, while maintaining confidential client case notes, history, and data through HMIS (Unity).
  • Oversee data quality of all client files upon entry and exit from the program.
  • Coordinate Sanctuary groups in Emergency Shelter.
  • Monitor monthly coordinated entry follow-ups and verify updates within the first week of each month.
  • Provide administrative support for quality assurance in residential program services.
  • Participate in staff and group meetings.
  • Organize and run meetings for child and family-related decision-making (e.g., Family Team Conferences, Red Flags, House Celebrations).
  • Maintain client case notes, history, and data, ensuring confidentiality.
  • Generate and compile client outcomes and reports within Clarity for funder reporting.
  • Maintain minimum weekly contact to track family progress.
  • Monitor grant-required monthly outcomes reports for residential programs, including Coordinated Entry, exit destinations, and benefits verification.
  • Provide training to interns on proper case management guidelines.
  • Facilitate internal staff trainings and team meetings as instructed by leadership.
  • Participate in monthly collaboration meetings with other program teams.
  • Visit and meet with community service providers and organizations beneficial to maintaining self-sufficient living.
  • Participate in Metropolitan Ministries’ holiday efforts and fund-raising events (e.g., Bridge Builders) as needed.
  • Meet personal goals and contribute to team and organizational targets/goals as stated in the KPI and annual plan.
  • Attend required staff meetings and ongoing trainings.
  • Complete other duties as assigned.

Requirements

  • Bachelor’s degree in Social Work or Human Services field preferred.
  • 3-5 years of Case Management experience.
  • Knowledgeable about homeless issues, with experience working with individuals with mental health/substance abuse issues or willingness to seek continuing education related to the homeless population.
  • Work towards obtaining licensed supervisor status to provide supervision to registered interns.

Skills

  • Bilingual (Spanish) preferred.
  • Ability to work within a multi-disciplinary team framework; strong interpersonal skills.

  • Demonstrates maturity and sensitivity to cultural and individual differences in clients served.

  • High degree of maturity and ability to work with low socio-economic and diverse populations facing crisis situations.
  • Ability to deescalate clients on the edge of verbal or physical violence.
  • Computer literacy with knowledge of Microsoft Word and Excel.
  • Ability to write reports and conduct/participate in meetings.

Physical Requirements

  • Adequate hearing and speaking ability to participate in oral communication.
  • Capable of working outdoors in sunlight and higher temperatures on occasion.
  • Physical, emotional, and spiritual stamina to handle job-related issues and stress.
  • Ability to lift 20 lbs. or more.

Other Requirements

  • Legal authorization to work in the United States.
  • Must pass required background pre-employment screening.
  • Ability to communicate effectively in English.
  • Valid driver’s license, clean driving record, and ability to qualify for the Ministries’ driving program.
